Health Promotion Worker
Wraparound Project for Older Carers
£22k per annum

Full Time 35 hours per week.

One year contract

In conjunction with the Centre Manager the post holder will take the lead in developing and delivering the Wraparound Project – a project designed to improve the health and well being of older carers living in socially deprived areas across Belfast.

The post holder will work with key delivery partners – such as the Belfast Health and Social Care Trust to identify and develop a programme of health improvement, particularly directed towards isolated older carers. Every older carer identified will be offered an individual Health Awareness Programme tailored to their own needs focusing on both their emotional and physical health. Older carers will be engaged on a one-to-one basis in their own homes.
